Team Roster

Each team must submit one (1) Team Roster with all the requested information. Teams will only be allowed to have a maximum of twelve (12) players on their rosters.

Individual Player Waiver

Each team must submit one (1) waiver for each player. Therefore there should be a total of one (1) waiver form per team. No individual will be allowed to play without signing the waiver form.

NOTICE!!!

  • All forms are due by Friday October 23 at 9:00 p.m. NO LATE SUBMISSIONS WILL BE ACCEPTED!!! 9:01 p.m. is late.
  • Captains must submit all waiver forms and the team roster as one package.
  • Forms can be turned in at Masjid As-Salam or Masjid Al-Noor.
  • All participants must bring a picture ID on the day of the tournament.
